To participate in the North District Trials, students MUST have the attached North District Permission form signed by:

1) Authorised School Delegate (eg. Principal, Deputy Principal or Sports-coordinator) AND

2) Parent/s or Caregiver/s

Students only need to bring the signed permission form and project consent form to the Trial. This permission form must be handed to the District Convener prior to the commencement of the trial.

IF NO TRIAL FORM – NO PARTICIPATION IN TRIAL IS PERMITTED.
